We work together as one…

There are numerous aspects that contribute to improving an individual’s health. One aspect that poses a difficulty to many is determining how and where to obtain resources that assist the community in ameliorating their health. As a result of this gap, our health advocates face a significant challenge in assisting the community with their needs. Efforts made to address these issues in a specific community are referred to as "community-based interventions." 

These interventions, ideally, do not attempt to help manage any condition in isolation, but rather view each person holistically, identify their needs and attempt to provide

support • through • collaboration

between the community, Wits Heath Hubb, and other identified stakeholders. When health advocates visit patients at home, they identify problems and try to match them with service providers to ensure that individual and community needs are met.

On a routine visit, one of our health advocates met a community member who had suffered from a stroke. After being discharged from the hospital, he was referred to the local clinic for physiotherapy and supply of a wheelchair.

However, the local clinic had no wheelchairs in stock, and the physiotherapists were unavailable. The patient was then referred to a community health centre.

Our team, working with partners in the community, were able to obtain a wheelchair and are in the process of locating a physiotherapist.
